Falls medical center graduates first class of care coordinators

Niagara Falls Memorial Medical Center has graduated its first class of care coordinators under a state Department of Health grant to prepare workforce candidates for the future of health care.

Class members are the first of 120 people who will get education in partnership with Niagara County Community College by way of this two-year project.

Care Coordination is an emerging health care field that promotes deliberately organizing patient care activities and sharing information among all who are involved with a patient’s care to achieve greater safety and effectiveness.

Graduates will work in places that include primary care centers, hospitals and behavioral health agencies.
