Panama man fatally injured in Amish buggy accident

PANAMA — Henry H. Koffman Jr., believed to be in his late 40s, of Panama, was fatally injured when the Amish buggy he was operating was struck by a car driven by Peggy J. Miles, of Panama, about 4:56 p.m. Thursday, according to the Chautauqua County Sheriff’s Office.

After the crash in the 1100 block of Weeks Road in Panama, four young men and women who were in the buggy with Koffman had to be taken to the Hamot Medical Center in Erie, Pa., for treatment of injuries. Koffman was pronounced dead at the scene of the fatal incident.

The sheriff’s office and the Chautauqua County Accident Reconstruction team continue to investigate the incident. Ms. Miles had not been charged with any crimes in connection the incident as of late Thursday evening, but the sheriff’s office confirmed it is working with information that she may have fallen asleep just before crashing into the Amish buggy.
