ElderWood Health Care at Wedgewood of Amherst received the 2011 Innovative Practice Award for its culture change program, "The Final Farewell," from the New York State Health Facilities Association. The goal of the program is to honor the lives of their residents and the close relationships that develop among caregivers, residents and families during the end-of-life process.
