NIAGARA FALLS – Niagara Falls Memorial Medical Center will host a community conversation about getting the community engaged in health care from 6 to 7:30 p.m. Tuesday in the hospital’s first-floor auditorium.

Comments from the public received at the forum, to be facilitated by the P2 Collaborative of Western New York, will become part of Memorial’s application to participate in the state’s Delivery Reform Incentive Payment program, aimed at reducing avoidable hospital use by Medicaid clients.

The first 20 people in attendance will receive $10 gift cards. Refreshments will be provided.
